Liability Waiver

Does any one have a copy/link to a liability waiver?
A friend of mine has a track at his farm and holds a recreational land use permit for it. He doesn't mind when other people ride there, but wants to have some kind of a liability waiver.
I figured I check in here as I am sure you might be able to help.
