Flat repair, on the trail

For those of you who carry equipment to make flat repairs on the trail, what do you carry? My main concern is pinch flats, but I want to be ready for anything. I assume the only repair for a pinch flat, is carrying a spare 21" tube? That way, it'll work for either the front or rear. That's a lot to carry in a fanny pack. I was thinking of carrying my 2 tire irons, and then a CO2 cartridge pump,and a few patches.

Thats about what I do except I put all of that stuff in a fender bag instead of on my body.

I still have a fanny pack but only carry my basic tools on it so that it's not so heavy.
