Bob, that's the ultra-rare "predolo", like a piccolo but larger to fit the ginormous hands of a Pred.

This picture is obviously doctored, there is no way that Pred had himself tattooed on his head at that age, that tatoo came 6 years after the picture was taken. I believe it was 1903. And that isn't a musical instrument, it's a vintage assault weapon.
