My '02 CR250 has:

My 38mm PWK has a 48 pilot and 180 main.
I'm at sea level here in Hawaii. At 2.75 turns out it is still a little rich.

I'm gonna drop to a 45 pilot and 175 main. Stock needle (for a 2000 CR250).

is it rich through out or just on the top end

Both....I actually don't mind running it slightly richer, the motor runs cooler and hopefully won't have to worry about seizing the motor leaning out.I'm still running the same plug from last year.knock, knock on wood......

175 main, 45 pilot, DGK needle with its clip in the middle position, adjust airscrew to suit. BraaaaaaAAAAAP! Crisp, clean-running power from just above seal level to above 5,000 feet. Pump gas isn't a problem but I prefer to run race gas, and I put many hours on a single plug. Easy starting, never fouled a plug, blah blah, rave, rave.
