I've read a lot of opinions, but has anybody actually done a bottom shock bearing modification that really worked?

What sort of mod are you talking? Zerk?
Not zerk .... I was thinking different bearing (bigger?) or sleeve. Something that would last longer.

Its not so much that they wear out, but that they are junk to begin with.

I was able to get two years out of mine by just running a marine grease instead. the problem is water.

Junk is a good word for the bushings. I bought a new bolt and bushing for my 88. The suspension and shock are different but basically the same idea. I measured .045" difference in the O.D.. of the bolt and I.D. of the bushing. These were NEW parts. Nothing like having a sloppy fit to start with.
