Clutch plates..

i have been having problems wif my clutch lately..
when i clutch in and kick first gear.. the bike would jerk a little to the front. but tis doesn't happen all the time.. onli happens sometimes after a hard ride..
Was wondering is that i sign that my clutch plates are wearing off?? or issit air in the hydraulic clutch system??
