My 2005 RMZ 450 still doesn't sound perfectly in-tune after changing the shims to the correct clearance, making sure the timing chain tensioner is working and setting the timing 100%. The bike starts fairly easy (5 kicks when cold, 1 - 3 kicks when hot.) Is there anything else I can check to get rid of the knocky noise in the head and get it more smooth?

It's the "normal" valve noise which I am not so sure about. Under full throttle the engine sounds much better, ie. much less knocking noise. We are 1600 meter above sea level and use 93 unleaded octane.

Take it to a bike shop and let a mechanic listen to it. If it is serious let them fix it before it gernades. DONT RIDE IT ANYMORE til you have it checked out. It will cost you a lot more to get it fixed if you continue riding it and the motor lets go. Right now major repairs like new piston or bottom end will be a lot more inexpensive while the motor is still in tact. (notic I didnt say cheap)
